Description
Provides one-time, 12-month grants to nonprofits and government agencies that provide direct services to older adults (age 55+) and caregivers within SCAN Health Plan service areas. Priorities include nutrition access and food security, emergency financial assistance, and other projects keeping older adults healthy and independent.
Eligibility
Application process is by invitation only with grant applications reviewed quarterly.
